*TMS eligibility and the patient-specific treatment schedule are determined after clinical evaluation. The $9,000 estimate reflects 36 completed treatments at $250 each: one mapping/first-treatment encounter and 35 subsequent treatments. The $250 treatment rate includes the routine treatment hood, device, staffing, and ordinary operating costs. The TMS evaluation is $395, and a separately furnished progress review is $275. Maintenance, retreatment, and services outside the written treatment schedule are separately itemized. Patients are not charged for treatments that are not performed.

Self-pay enrollment is voluntary.

Professional fees exclude laboratory tests, medications, supplements and peptides unless specifically stated.

Patients receive a patient-specific Good Faith Estimate before scheduled self-pay care.

Clinical eligibility and recommendations are determined individually; outcomes are not guaranteed.

A signed service agreement and any required clinical consent are completed before treatment.

Frequently Asked Questions

Questions Before You Begin?

Can I use insurance?

Some medically necessary services may be eligible for insurance billing, depending on your plan, benefits and the service provided. Self-pay programs are financially separate from insurance-based care and do not replace deductibles, copayments or coinsurance. Coverage and prior authorization are not guarantees of payment.

Do I have to complete every laboratory test?

No. The physician recommends testing based on clinical judgment, and you may decline a recommended test after discussing how a narrower evaluation could limit interpretation. Each ordered test is separately itemized before ordering, and you are not charged for a test that is not performed.

What happens if I stop a program early?

You may cancel a nonrecurring program by written or electronic notice. Any refund is determined from the amount paid after subtracting completed services at the disclosed standalone value and any authorized, nonrecoverable third-party costs already incurred. The signed service agreement and Good Faith Estimate control.

Which forms will I sign?

Depending on the service, you will receive the Master Patient Financial and Practice Services Agreement, the applicable program addendum, a patient-specific Good Faith Estimate and any required clinical informed consent. Memberships also require explicit recurring-payment authorization.
